Outreach Worker JCTOD Outreach Inc 1415 Oriskany Street West, Utica, NY 13502 Job Overview We are seeking a dedicated and compassionate Outreach Worker to join our team. The ideal candidate will play a crucial role in connecting individuals with essential services and resources, particularly in the areas of healthcare, addiction counseling, and social support. This position requires strong communication skills and a commitment to educating the community about available programs, including Medicare and other health-related services. Responsibilities Educate individuals and families about Medicare options and other healthcare services available to them. Conduct data collection to assess community needs and track outreach efforts effectively. Provide addiction counseling support to individuals struggling with substance use disorders. Collaborate with healthcare professionals, including nurses, to ensure comprehensive care for clients. Engage in public speaking events to raise awareness about services offered and promote community health initiatives. Utilize medical terminology effectively when communicating with clients and healthcare providers. Work closely with individuals with disabilities to provide tailored support and resources. Assist clients in navigating social work systems and accessing necessary services for their well-being. Experience Previous experience in outreach, social work, or a related field is preferred. Familiarity with Medicare processes and healthcare systems is advantageous. Background in addiction counseling or mental health support is a plus. Strong public speaking skills and the ability to communicate effectively with diverse populations are essential. Experience working with individuals with disabilities or understanding their unique needs is highly valued.
