Key Responsibilities:
Plan, organize, and oversee the Social Services Department. Ensure all resident social, medical, and emotional needs are met in compliance with state and federal regulations. Develop individualized care and discharge plans. Conduct resident assessments and maintain accurate documentation. Maintain strong partnerships with medical professionals and community resources. Support residents and families through transitions and care planning. Uphold resident rights, privacy, and confidentiality at all times. Lead department staff and manage budgets and departmental goals.
Qualifications:
Experience in Social Work or a related field preferred. Experience in long-term care or healthcare setting strongly preferred. Excellent communication, leadership, and organizational skills.
