Key Responsibilities:

Conduct psychosocial assessments and develop individualized care plans Provide emotional support and counseling to residents and families Assist with care planning, discharge planning, and community referrals Collaborate with nursing and interdisciplinary teams Ensure documentation and services meet state and federal regulations

Qualifications:
