Social Services Director Lake Montgomery Nursing & Rehabilitation Center 1270 SW Main Boulevard, Lake City, FL 32025 Job Description Lake Montgomery Health and Rehabilitation Center is now hiring a full-time Director of Social Services. The Director of Social Services oversees the overall operation of the Resident and Family Services Department; organizes and plans Social Service Programs for residents and patients based on their individualized needs within the facility; coordinates activities with the community, outside facilities and agencies that can provide economic, psychological, and medical assistance; and helps the resident, patient, staff, and families with issues that impacts or prevents adjustment or any other aspect of the resident’s quality of life. Also serves as the facility Grievance Coordinator to help facilitate resolution of customer complaints and grievances; and assists the Administrator with Abuse Prevention and Investigations and is one of the back-up reporters for filing any reportable incidents to DCF and AHCA. DIRECTOR OF SOCIAL SERVICES QUALIFICATIONS
Must have Bachelor of Science degree in social work or related human science field; a Master’s degree is preferred.
