- Nursing Home Are you a compassionate and dedicated Social Worker looking to make a meaningful impact in the lives of seniors? A leading Nursing Home in your area is seeking a Director of Social Services to enhance the well-being and quality of life of our residents. Key Responsibilities Resident Assessments
- Evaluate the social, emotional, and psychological needs of residents and their families. Individualized Care Plans
- Develop customized care plans to address each resident’s unique needs. Interdisciplinary Collaboration
- Work closely with nurses, therapists, and other healthcare professionals to ensure seamless coordination of care . Emotional Support & Counseling
- Provide guidance and counseling to residents and their families during challenging transitions . Community Resources
- Connect residents and families with support services and community resources to enhance their quality of life. Resident Advocacy
- Ensure that residents’ rights, preferences, and dignity are upheld.
Qualifications Bachelor’s Degree in Social Work (BSW) or a related field ( not required but preferred ). Strong communication and interpersonal skills.
