Conduct comprehensive interviews with residents and families to gather detailed social histories and ensure personalized care plans. Collaborate with residents and families to develop and implement customized social service programs. Identify and address social, emotional, and environmental challenges, offering solutions like financial assistance, discharge planning, and coordination with community organizations for specialized services. Partner with the administration, medical, nursing, and other departments to design and implement comprehensive social services programs and engaging activities. Participate in facility surveys and inspections by authorized government agencies, ensuring compliance with quality standards. Advocate for residents by effectively communicating their social, psychological, and emotional needs to the care team, including physicians and medical professionals.
Requirements:
Minimum of 3 years of experience in social services, preferably in a skilled nursing facility (SNF). Strong interpersonal, communication, and problem-solving skills.
