The successful candidate will be responsible for the provision of services for the highest psychological and social well-being of the residents and all other duties assigned. The Director of Social Services is an advocate for the residents and is committed to sustaining an environment that humanizes and individualizes each resident.
Responsibilities:
Lend continued assistance and support to residents from their time of admission, to discharge or transfer. Assisting the residents and families with personal and social needs and concersns related to institutionalization or adjustment to change in medical condition. Assessment of new residents to evaluate appropriateness of placement. Take an active role in disseminating information to residents and their families regarding, but not limited to, the following areas: Interpretation of Resident Rights Classifications of residents according to Department of Health Regulations (ie, Level
II PASRR
regulations for serious mental illness/developmental disability) Advance Directives and Family Health Care Decisions Coordinate and monitor needed available services for residents to assure optimum level of emotional, physical, psychological well-being and independence. Member of Quality Assurance team, prepare QA reports and presents in meetings. Help formulate Comprehensive Care Plans for resident and assist with discharge planning needs. Member of Comprehensive Care Plan team that addresses peer to peer altercations and incidents within the facility. Involve the residents with other department or Administrative needs, as appropriate, regarding such matters as bed retention, room change, transfer, and discharge. Facilitate communication with other disciplines. Provide documentation in charts in accordance with departmental, facility, state and federal regulations. Facilitate investigation and resolutions of grievance and complaints. Experience Degree in Social Work Preferred Long-Term Care Experience a Plus Qualifications Supervising experience Social work background Management skills Program development expertise Experience working with individuals with developmental disabilities Relationship management abilities Behavioral health knowledge
