SERVICES DIRECTOR
Main Duties:

The primary purpose of your position is to plan, organize, develop, and direct the overall operation of the Facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines, and regulations, our established policies and procedures, and as may be directed by the ED/Administrator, to assure that the medically related emotional and social needs of the residents are met and maintained on an individual’s basis.

Skills and Responsibilities:

Develop and implement policies and procedures for the identification of medically related social and emotional needs of residents. Participate in community planning related to the interests of the Facility and the services and needs of the resident and family. Coordinate discharge planning, development and implementation of social care plans and resident assessments. Utilize facility discharge planning form to facilitate a smooth transition from the facility to discharge placement with DME, home health and any other community support services as necessary. Coordinate facility abuse, neglect, exploitation and misappropriation of property prevention program. Provide in-services to all facility personnel as directed by the ED/Administrator. As the facility designated Abuse Coordinator, investigate all allegations in a timely manner, report allegations within regulatory time frames, provide report to ED/Administrator, and Director of Nursing. Must be knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.

Education and Experience:

A Master’s Degree in social work preferred. Must possess, as a minimum, a Bachelor’s Degree in social work and at least 1-2 years of experience in a LTC/SNF setting.
