Job Description:
Bedrock Rehabilitation and Nursing Center at Wedgewood is seeking a compassionate and experienced Director of Social Services to lead our team in providing outstanding care and services to our residents. The Director of Social Services will be responsible for planning, developing, implementing, and evaluating the social service programs within the facility, ensuring the well-being and support of residents and their families.
Key Responsibilities:
Plan, develop, organize, and direct social service programs in the facility. Interview residents or family members to obtain social history and assess needs. Provide information to residents and families regarding Medicare/Medicaid and other financial assistance programs. Refer residents and families to appropriate external social service agencies when necessary. Assist in improving workflow efficiencies across departments, reducing costs, and eliminating waste. Develop and implement departmental policies and procedures. Review and update departmental job descriptions annually. Arrange transportation to other facilities as needed. Ensure social service progress notes are clear and descriptive of services provided and residents’ responses. Coordinate with other departments to support social services activities. Attend quality assurance and performance improvement (QAPI) meetings, helping to develop and implement corrective actions. Maintain a community and social services referral file with relevant agencies and organizations. Interpret social services policies and procedures to employees, residents, visitors, and government agencies. Stay informed on current federal and state regulations and professional standards, recommending changes to the Administrator.
Requirements:
A minimum of a bachelor’s degree in social work or a human services field (such as sociology, gerontology, special education, rehabilitation counseling, or psychology). Master of Social Work (MSW) or related field preferred. At least two (2) years of social work experience in a healthcare setting, working directly with individuals. One to two (1-2) years of supervisory experience preferred.
