Job Description
Director of Social Services Confidential Canton, GA 30115 Are you a compassionate leader with a strong background in social services and a passion for serving individuals with developmental disabilities? We are seeking a Director of Social Services to lead and elevate our support programs. This is a key leadership role responsible for shaping and driving person-centered services that improve quality of life and ensure regulatory excellence.
Key Responsibilities:
Oversee all aspects of social services for a diverse resident population in a skilled nursing environment. Support residents and families through admission, care planning, and discharge processes. Coordinate psychosocial assessments and care plans in compliance with regulatory standards. Supervise and mentor social service staff to ensure compassionate and high-quality care. Serve as a liaison between residents, families, medical staff, and community resources. Manage the department’s documentation, including MDS and care conference notes. Ensure compliance with federal, state, and local regulations, including CMS requirements. Advocate for residents’ rights and support residents in addressing emotional or behavioral needs.
Qualifications:
Bachelor’s degree in Social Work or related field (Master’s degree preferred). Minimum 2 years of experience in a nursing home or long-term care setting . Previous leadership or supervisory experience in social services strongly preferred. Strong understanding of Medicare/Medicaid, MDS, discharge planning, and psychosocial care. Excellent interpersonal, communication, and organizational skills. Passion for working with the elderly and supporting residents with complex needs.

$70,000.00 – $80,000.00 per year
