Full Job Description Position Summary:
Orchard View Rehabilitation and Skilled Nursing Center, a proud member of the Hospital Authority of Columbus, GA, is seeking a compassionate, organized, and dedicated Social Service Director to join our leadership team. This position plays a vital role in ensuring the emotional and social well-being of our residents, serving as a key liaison among residents, families, medical staff, and community resources.
Key Responsibilities:
Oversee and manage all social services functions in compliance with federal, state, and local regulations. Conduct psychosocial assessments and develop individualized care plans for new and existing residents. Serve as an advocate for residents’ rights and needs within the facility. Coordinate discharges, transfers, and follow-up care with families and external agencies. Provide support and counseling to residents and families adjusting to life transitions. Collaborate with interdisciplinary teams to ensure holistic care delivery. Supervise and train social work staff, ensuring ongoing professional development.
Qualifications:
Bachelor’s degree in Social Work or a related field required. Current state licensure or certification in Social Work (LBSW or LMSW in Georgia) preferred. Minimum of 2-3 years of experience in long-term care setting. Strong interpersonal, communication, and organizational skills. Familiarity with Medicare/Medicaid regulations and discharge planning preferred. Why Join Us?
