Entire Facility PURPOSE:
The purpose of this position is to provide overall supervision and oversight to the Social Services Department and to meet the social and/or emotional needs that affect the residents ability to achieve their highest practicable psychosocial well being; participate in the development of residents comprehensive care plans; follow policies and procedures to provide social services to residents in compliance with federal, state and local regulations. Hours of work are 0 to 80 hours per pay period plus or minus as business needs and assigned by the Administrator.

EDUCATION AND QUALIFICATIONS
Bachelors Degree in Social Work or a Bachelors Degree in a Human Services field including but not limited to Sociology, Special Education, Rehabilitation Counseling, Psychology or Gerontology. One year of supervised Social Services experience in a health care setting.
REQUIRED DUTIES
: Plans, develops, organizes, implements, evaluates, and directs (directors position only) the Social Services Department in accordance with current state, federal and facility standards. Maintains policies and procedures to assure that the medically related emotional and social needs of the residents are met/maintained on an individual basis. Must be able to help transport residents to hospital or medical facility in emergency. Reports all incidents immediately. Must be alert to all residents change in condition. Must have good vision, manual dexterity and ability to push wheelchair and reposition residents,
DUTIES:
Plans, develops, organizes, implements, evaluates and directs the Social Services Department in accordance with current state, federal and facility standards. Identifies the medially related social and emotional needs of the resident and arranges for such needs to be met by a qualified staff member of the facility or by referral to appropriate agencies. Refers residents and/or families in need of financial assistance to the appropriate agency. Maintains records of all resident interviews and follow-up action taken. Consults with Administrator, Director of Nursing and other Supervisors to relate the functions of the department to other nursing facility activities. Suggests improvements in nursing home procedures that will make the residents association with the nursing facility less complicated and more pleasant. Provides guidance and leadership for carrying out the discharge plan. Schedules and conducts facility orientation with resident and/or family members and other department heads. Participates as a member of the residents Care Plan Team and notifies family and resident of Care Plan Conference schedule. Informs each resident of his/her rights and follows through to see that those rights are being carried out. Also informs resident of his/her responsibilities. Provides information to residents on Advance Directives in accordance with the Patient Self-Determination Act. Ensures completion of and/or compiles a Social History of each resident and updates as necessary. Maintains Social Work Progress Notes by recording all significant contacts made in the course of performing the service. Provides emotional support to families regarding residents condition on an ongoing and crisis basis. Available to families/residents on an ongoing basis to answer questions regarding facility services and policies. Makes referrals to community agencies regarding Medicare/Medicaid eligibility. Attends and participates in QA Committee meetings. Develops and maintains a close working relationship with public and private agencies, i.e. Department of Social Service, community hospital, mental health clinic, Community Long Term Care, other nursing facilities, etc. Keeps current on developments and achievements in medical social service professions. Reviews and approves weekly timecard facsimiles for department staff. Completes performance evaluations and review with department staff in a timely manner. Assist as needed in transportation needs of residents which includes, making appointments, arranging with family, facility or public transport. Responsible for discharge planning. Other duties as assigned by the Administrator. #sj
