Duties include, but are not limited to: Admission and Ongoing Assessments/Documentation Helping with Adjustment MDS 3.0 Care Planning Assisting with Medicaid Applications and other Financial Matters Advocacy Crisis Intervention Discharge Planning To qualify for this position, the ideal
Candidate:
Must have a NJ Social Work Certification CSW/LSW License in good standing Must be able to multi-task and organize Must have a passion for the senior population Must have strong writing skills Must be an effective communicator Must be a team player Familiarity with Point Click Care (PCC) is a plus but not a requirement.
