Administrator SUMMARY Under direct supervision of Administration, the Director of Social Work shares the responsibility of providing for the medically related social services needs of each resident and assures that those needs are met. He or she assists with arranging services to meet the needs of residents; providing liaison between the resident, family, facility staff and the community.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following. Other duties may be assigned. Manages a team of social workers Communicates with physicians, nurses, social workers, residents, and families to determine those physical and emotional needs necessary for the appropriate placement and minimize trauma surrounding transfer and placement. Obtains social history for each resident establishing goals with family and/or resident. Participates in on-going evaluation of resident’s needs and goals through multi-disciplinary staff meetings and formulates care plans Provides counseling and/or referral to appropriate sources as problems are identified. Facilitates discharge planning. Works with each resident toward restoring the resident’s social functioning to the fullest extent possible. Makes arrangements for obtaining needed adaptive equipment, clothing, and personal items. Work in unison with all other departments toward the improvement of the physical, social, emotional and spiritual aspects of the resident’s life. Assists residents with financial and legal matters such as applying for pensions, referrals to legal aide, referrals to funeral homes for pre-planning arrangements. Act as an advocate for residents and families who need to work with outside community resources. Assist with other duties as assigned by Administration.

The requirements listed below are representative of the knowledge, skill, and/or ability required. Must meet the current rules and regulation requirements for the position including a Bachelor’s degree in social work or a related human services field, and 1 year supervised social work experience in a healthcare setting. 2 years experience in sub-acute rehabilitation and long term care.
