Demonstrates critical thinking in the identification of social and safety issues that may occur during the course of residents’ stay. Demonstrates initiative and seeks formal and informal opportunities to improve practice. Develop and implement written, interdisciplinary care plans for residents from admission to discharge. Organize and facilitate meetings for residents’ families to improve residents’ overall well-being.
