Job Duties:
The CFTM Facilitator will hold Child and Family Team Meetings at critical decision points to support the overall goals of Child Welfare Services.
Provide Child and Family Team Meeting services through preparation, planning and facilitating CFTM’s. Including on call CFTM facilitation.
Engage families in the family finding process to include Genograms, Ecomaps, Connectedness Charts and CLEAR or Seneca Searches. Ensure that families have supports and kin present at the table for the Child and Family Team Meeting.
Document CFTM’s accurately in CAPSS and other mechanisms.
Increase community and provider network awareness of DSS family engagement practices such as child and family teamings.
Ensure that Personal Identifying Information (PII) is properly used, accessed, gathered, shared and disposed. Protect the agency networks and applications by safeguarding systems, equipment and data.
Perform other duties as assigned
Minimum and Additional Requirements
A Bachelor’s Degree in Social Work, Social Welfare, Counseling, Behavioral Science, or Human Services or a Bachelor’s Degree in any other field and three (3) years of professional experience in human services or social service programs.
A Master’s Degree in Social Work, Social Welfare, Counseling, Behavioral Science, or Human Services; or a Master’s Degree in any other field and once (1) year of professional experience in human services or social service programs.
