Must have BSW or MSW Must have experience as a Director of Social Services in an LTC/ nursing home setting Must have some knowledge and understanding of PDPM Must have strong organizational and case management abilities Strong leadership and management skills Strong verbal and written communication skills

Job Responsibilities:

Responsible for coordinating all admissions and discharges to ensure the facility census is kept at its maximum capacity. Conducting initial social and emotional assessments of new residents to understand their needs, support systems, and desired level of involvement in activities. Regularly communicating with residents’ families, providing updates on their loved one’s well-being, addressing concerns, and facilitating family meetings. Identifying and connecting residents to appropriate community resources such as support groups, volunteer services, and transportation options Organizing and facilitating social activities and events that cater to diverse resident interests, promoting socialization and engagement Acting as an advocate for residents by ensuring their needs are met and addressing any concerns related to their social or emotional well-being Assisting with discharge planning by coordinating with healthcare providers, family members, and community services to ensure a smooth transition upon leaving the facility Participates in an interdisciplinary team approach to better serve the needs of the facility population. #7119
