Position Overview:
The Director of Social Services functions to plan, develop, organize, implement, evaluate, and direct social service programs in accordance with current federal, state, and local standards, guidelines, and facility policies and procedures to ensure that the medically related emotional and social needs of each resident are met and maintained on an individual basis. Assist in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility. Participate in discharge planning, development and implementation of social care plans and resident assessments. Interview residents/families as necessary. Record and maintain regular Social Service progress notes indicating response to the treatment plan and/or adjustment to institutional life. Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident. Maintain contact with the resident’s family, involving them with non-medical progress reports as necessary. Assist in resolution of grievances as voiced by resident, family, responsible party or ombudsman.
Qualifications:
Must possess, as a minimum, a bachelor’s degree in social work or a bachelor’s degree in a human service field, including but not limited to sociology, special education, rehabilitation counseling, and psychology. Must have, as a minimum, one year of supervised social work experience, preferably in a skilled nursing setting.
