Job Description

Community Liaison 3.9 3.9 out of 5 stars 22 Danbury Road, Wilton, CT 06897 Responsible for generating referrals for Waveny’s home care and hospice organizations by building relationships with physicians and hospital systems. Responsibilities include market analysis, sales strategy development, goals and plans, sales calls, call reports and evaluating results and effectiveness of sales activities. This position will support business development activities and establish strong relationships with new and existing referral sources in coordination with the Care Transitions Coordinator. Functions as primary external educational resource for community members relative to Waveny’s home care organization and skilled nursing facility’s programs/services and community resources.

Qualifications :

1. Experience in health care sales/marketing, pharmaceutical sales, home care and/or skilled nursing sector preferred 2. Superior communication and organizational skills 3. Superior customer service skills 4. Computer proficiency 5. Presentation proficiency 6. Bachelor’s degree

Responsibilities :

¨ Develop and implement a structured and systematic sales plan in accordance with Waveny LifeCare Network’s strategic priorities, with a particular focus on physicians and hospital systems. ¨ Develops and maintains database and statistics related to contacts and activities. ¨ Analyze market opportunities, make recommendations and create/implement an appropriate plan. ¨ Market Waveny’s home care and hospice organizations and their offered services through direct sales approach to new and established referral sources and the community at large ¨ Coordinate activities with other network staff/departments (Marketing/Development, Community Health, Hospice and Home Care) to ensure broad-based and coordinated approach and to combine resources, when possible. ¨ In coordination with the Care Transitions Coordinator, work to establish and maintain professional relationships with physicians, case managers from area hospitals, human/social service directors, and other new and established referral sources. ¨ Create and maintain electronic marketing and social media tools and communications, as directed ¨ Assist in web-site maintenance and updates ¨ In concert with organizations intake departments and the Care Transitions Coordinator, serve as a community resource and referral source for anyone contacting the Agency. ¨ Researches educational needs of referrals sources and prepares and delivers pertinent presentations on a regular basis. ¨ Participates in agency internal and external events, as requested ¨ Maintains productivity expectations of 15-20 contacts each week.
