The Social Services Specialist responsibilities include: Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
Provides coordination, referral and interdisciplinary discharge planning
They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
Possesses a clear understanding of discharge and transition planning and linkage with community resources
Demonstrates skills in planning, organizing, and managing multiple functions Position details:

Status:
PRN (as needed), hourly
Schedule:
8:00 AM – 4:30 PM, days vary
Weekend Requirement:

No Certifications and Licensures:
Preferred ACM (Case Management) or
Preferred CCM (Case Manager)
Education:
Required Bachelors in Social Work, Sociology, or Related Field
Preferred Masters Degree in Social Work, Sociology or Related Field (If MSW level, will be hired as Social Services Specialist II)
Experience:
Required 6 months Internship or
Required 6 months Social Work in an acute care setting
