Job Description
Service Coordinator for Southern Villas Residential Complex St. Johns County Council on Aging, Inc. Saint Augustine, FL 32084 Provide general services and case management for residents at a senior residential complex. Support elder residents in obtaining services to improve quality of life and support aging in place. Facilitate community building opportunities for residents and foster a positive environment of inclusion.
Hours:
Part-time (Monday, Wednesday, Friday). 20-24 Hours Responsibilities include : Provide general service management which includes intake, education (service available and application procedures) and referral of residents to service providers in the community. These social services may include Meals on Wheels, transportation, home health aides, homemakers, financial assistance, counseling, preventive health screening, and other needed services. Develop and maintains a current Resource Directory. This directory will include a comprehensive listing of available local social services Plan and execute a monthly calendar of educational events Monitor ongoing provision of services from community agencies Network and serve as a liaison to community agencies Assist residents in building informal support networks Referrals to Adult Protective Services Perform other duties as necessary Training and experience requirements: Two or three years of experience in social services delivery with elderly and/or disabled populations. Bachelor’s Degree in Social Work or related field preferred, but not required. Demonstrate working knowledge of supportive services and other resources in St Johns County. Demonstrate the ability to advocate, organize, problem solve and provide results for the residents served. Be able to relate to and communicate with functionally impaired seniors of many different backgrounds. Strong written communication skills, including ability to effectively communicate with colleagues, participants, and community partners. Excellent computer skills in MS Office (Word and Excel), and G Suite Tools (Gmail, Drive, Docs, Sheets, and Calendar). A valid driver’s license Must pass a Level 2
