Description New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. In addition, New Horizons assists 22,000 students in area schools, and we work closely with the courts, law enforcement, jails, and hospitals to help improve the health of individuals and the quality of life in our community. New Horizons is a nonprofit agency seeking a caring and compassionate Housing Specialist to Assist clients in finding and securing living arrangements in the community at fair market rental rates. As a Housing Liaison your responsibilities will include: Assist clients in finding and securing living arrangements in the community at fair market rental rates
Enroll clients in Client Track System/HMIS
Secure representative payees for clients who require that service
Monitor clients in housing to assure that they successfully sustain the housing provided through the Project
Meets with referred individuals to assist with the application for assistance
Track client participation in required medical, mental health and other supportive services necessary to address their disability and housing needs/Case Management
Participate regularly in Shelter Plus Care Team Meetings conducted by the Treasure Coast Homeless Services Council, Inc.
Participates in administrative committees and meetings, clinical training, staff development and community relation activities Requirements New Horizons will need you to have the following qualifications: Bachelor’s Degree from an accredited university or college with a major in the field of counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field OR a Certified Behavioral Health Technician and five (5) years’ behavioral healthcare experience
