SERVICES PROFESSIONAL JOB DESCRIPTION
Job Summary:
The Human Service Professional (HSP) is responsible for making sure the residents’ educational, emotional, social, medical and other needs are met. The HSP is responsible for coordinating and providing all services for residents.
Direct Supervisor:
Director Job Duties & Responsibilities:
Monitors and manages resident records and ensures all documentation is accurate and up-to-date. Develops, completes, reviews, processes, files and submits documents and reports to all appropriate parties i.e. Case Manager, ILP Coordinator, Courts, Providers, Schools and all the Community Supports. Ensures all information is updated in GA Score and Shines. Ensures that all timelines and deadlines are met for all required documentation. Ensures residents’ files are kept confidential and secured at all times. Transports residents to appointments, meetings, court etc., as needed (HSP must ensure all proper documents are transported and returned in the event other staff member transports resident). Attends all meetings and IEPs concerning residents and acts as a liaison with case managers, probation officers, and other stakeholders. Fulfills Release of Information requests, ensuring proper authorization is in place. Creates partnerships and maintains support through community affiliations. Creates and implements assessments and services for all residents. Maintains strong knowledge and application of all state, licensure, and OPM guidelines. Submits documents in a timely manner and communicates effectively to all involved parties. Maintains strong knowledge and application of appropriate developmental treatment programs, evaluations, observation and techniques and makes recommendations as appropriate. Intervenes and de-escalates crisis situations when needed. Acts as a team player at all times. Attends all staff meetings, participates in scheduled training sessions and on the job training. Ensures annual training requirements are met. Supports and abides by all company policies and procedures. Reports all incidents and occurrences within a timely manner. Performs other duties as assigned.
Education and Experience Required:
Bachelor’s degree or higher from an accredited college or university in the area of behavioral or social sciences, social work, psychology, childhood education and a minimum of two years’ experience in the field of social services or human services Must have a satisfactory DMV record and criminal background check.
Essential Performance Requirements:
Must be proficient in GA Score and GA Shines and in uploading documents into both systems. Must have strong organizational skills and time management skills. Must be able to communicate verbally and in writing with coworkers, residents, and the public. Must have the ability to work well under pressure. Must be able to safely operate a vehicle, have a valid driver’s license and current auto insurance.
Working Conditions:
Care is provided to residents in the comfort of CLSP homes. Must be able to be on call 24/7 to assist staff in emergency situations. Staff are expected to participate in continuing education to build their knowledge, skills and abilities. Staff are required to maintain a satisfactory DMV record and background check throughout their employment as well as current CPR and First Aid certification.
