Job Description

Pay Grade:

D
Under direct supervision, provides a broad range of paraprofessional social services to clients and their families. Provides information to clients, conducts intakes or assists staff in the delivery of social services.

Job Responsibilities:

Ensures database information is maintained on case intake and dispositions.
Assists clients in locating and utilizingmunity resources including legal, medical, financial assistance, and other referral services.
Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client’s overall progress.
Provides short-term case management and referral services to clients with emergency situations.
Routinely enter data into automated system in order to add or update member records.
Minimum Qualifications
High school diploma/GED and one (1) year of experience in a social services-related position.

Preferred Qualifications:

Preference will be given to candidates who, in addition to meeting the minimum qualifications, demonstrate some or all of the following skills/experience. Strong Dept of Family and Children Services programs and process knowledge and experience.
DFCS social services system knowledge (SHINES).
Highlyanized and self motivated.
Ability to provide transportation and evidence of a valid driver’s license for required travel throughout the state.
