Minimum Qualifications Five years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR Six years of full-time work experience in any field plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR A bachelor’s degree plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR An advanced degree or a Juris Doctorate plus one year of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.

EXPERIENCE SUBSTITUTION

Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field. Job Duties and Other Information Job Duties:

60% Resource Coordination and Support Conducts comprehensive assessments of participant needs and preferences to identify appropriate resources needed for successful transition to permanent supportive housing. Monitors participants to follow up on identified resource solutions. Ensures all applications, forms, and other supporting documentation needed to secure housing and other identified community resources are received and processed in a timely manner, meeting all resource/referral guidelines and deadlines. Monitors housing availability and challenges. Identifies solutions for unique challenges such as limited access to utilities, septic systems, and internet connectivity. Addresses barriers for rural area resource needs including limited housing stock, distance and transportation, and availability of support services. Addresses barriers for urban area resource needs including higher cost of living challenges, diverse housing options, and increased competition for support services. Identifies and monitors resource options to present to the participant to ensure a person centered focus on housing and resource utilization preferences. Assists PSH participants with navigating complex referral options to facilitate a successful transition to permanent supportive housing. Completes necessary applications for identified housing and resources, and monitors. Participant referrals to community partners and/or programs. Tracks, monitors, and assists with the submission of required annual recertification process for all PSH participants. Performs inspections to ensure properties meet HUD guidelines regarding safe, sanitary, and decent conditions for tenants 25% Community Outreach Identifies community resources and communicates with supervisor on new community partnerships that provide supports to those transitioning into permanent supportive housing. Advocates on behalf of clients to ensure access to necessary community supports and resources, including housing subsidies, rental assistance, and public benefits. Presents PSH information to community partners and to relevant agencies. Utilizes housing policy knowledge to represent OAAS at agency housing related meetings including, but not limited to My Choice Louisiana, My Place Louisiana, Local Housing Coalition Agency meetings, and Louisiana Housing Corporation meetings. Identifies and builds collaborative relationships with community partners to facilitate permanent supportive housing transitions. Identifies and monitors resource and referral opportunities 10% Program Integrity Provides training and in-service to regional employees on best practices and procedures related to resource utilization to ensure successful transition of participants into permanent supportive housing. Monitors post pre- and post-transition housing support and services including landlord communication, training and support on payment of utilities, and follow up on any support requests. Monitors current policies and or initiatives to apply to current application/processes as well as to make suggestions to improve current PSH policies and procedures. Enters all information required for tracking PSH program outcomes and deliverables to state and federal partners in appropriate systems (i.e. Yardi, Monday.com, SharePoint, etc). 5% Emergency Functions Fulfills obligation for State’s emergency preparedness and response efforts as assigned during an emergency and assists impacted participants and providers to assure they are back in operation as soon as possible following the event. 5% Other Statewide travel as required to complete tasks associated with the effective operation of OAAS.
