For the last 20+ years ACTS Career Center has been a trusted resource to each community and serves to expand and enhance the support offered to job seekers. The ACTS Career Center is seeking a Case Management Coordinator. The Case Management Coordinator will work closely with the ION Program Director and the Executive Director of ACTS Career Center to ensure programmatic services and comprehensive case management are provided to ION program participants. The Case Management Coordinator will be responsible for overseeing the case management processes for individuals enrolled in ACTS Career Center’s ION (Increasing Opportunities Now) program for justice involved youth and young adults, ages 18-24. This role involves carrying a small caseload of 10-20 clients and ensuring effective coordination of case management services to achieve clients’ career development goals. The Case Management Coordinator will work closely with program participants, staff, and community partners to deliver high-quality services, ensure compliance with REO and WIOA reporting requirements, and maintain detailed documentation of case management activities.

Key Responsibilities:
Case Management Oversight:

Manage a caseload of 10-20 clients, providing individualized case management services, including assessments, goal setting, resource referrals, and progress monitoring. Develop and implement client action plans, ensuring they are tailored to individual needs and aligned with career goals. Conduct regular follow-ups with clients to ensure their needs are being met and monitor their progress toward achieving employment and educational goals. Will supervise two Case Managers.

REO and WIOA Reporting:

Ensure all client records are maintained in accordance with REO (Reemployment and Eligibility Assessment) and WIOA (Workforce Innovation and Opportunity Act) reporting requirements. Prepare and submit required reports on time, ensuring accuracy and completeness. Monitor client eligibility and participation to ensure compliance with all relevant programmatic policies and regulations.

Program Coordination:

Provide oversight and guidance to staff members involved in case management processes, ensuring consistency in service delivery. Collaborate with other team members to ensure a holistic, client-centered approach to service delivery. Evaluate the effectiveness of case management services and make recommendations for improvements when necessary.

Client Advocacy and Support:

Act as an advocate for clients, ensuring they have access to necessary resources and services that support their career goals. Establish and maintain relationships with community partners and employers to facilitate employment opportunities for clients. Offer guidance and support to clients in overcoming barriers to employment, such as childcare, transportation, and financial challenges. Will occasionally assist the Case Managers with class facilitation.

Qualifications:
Education:

Bachelor’s degree in Social Work, Human Services, or a related field.

Experience:

A minimum of 5 years of experience in a nonprofit setting, with at least 2 years in a supervisory or leadership role. Strong knowledge of case management practices and familiarity with workforce development and employment support services. Experience with WIOA, REO, and other funding or reporting requirements is strongly preferred.

Skills & Abilities:

Ability to manage a caseload and prioritize tasks effectively. Strong organizational, communication, and interpersonal skills. Ability to maintain confidentiality and adhere to professional ethical standards. Proficiency in Microsoft Office Suite (Word, Excel, Outlook). Ability to work independently and as part of a team. Strong problem-solving skills and the ability to adapt to changing needs and priorities.

Physical Requirements:

Ability to sit for extended periods, use a computer, and communicate with clients in person and via phone/email.
