GENERAL STATEMENT OF JOB
The Housing Specialist is responsible forthe completion of administrative work directly related to themanagement of identified housing programs and initiatives aimed atsupporting individuals who experience Mental Health, Substance Use,and/or Intellectual and Developmental Disabilities to live in themost integrated setting appropriate to their needs. The employeeperforms a variety of functions related to housing including theidentification of social and economic opportunities that are likelyto enhance health outcomes for individuals served. The employeefocuses on connecting individuals to needed services, supportivehousing opportunities and other social determinants of healthresources. This is accomplished by developing the means to addresshousing deficits and maximizing opportunities for members servedthrough housing navigation, housing development, servicedevelopment, landlord engagement, community outreach activities,and increase collaboration with internal and external stakeholders.
The employee will provide expertise, guidance, and training forinternal and external stakeholders as it relates to accessingappropriate housing resources and supports for those served. Theemployee ensures that engagement with identified contractedproviders is consistent and in line with contract obligations. Theemployee, in collaboration with assigned Supervisor, ensures thatall housing supports activities are delivered in accordance withthe terms and conditions of the

PHYSICAL REQUIREMENTS
Close visual acuity to perform activities suchas preparation and analysis of documents; viewing a computerterminal; and extensive reading. Physical activity in this positionincludes crouching, reaching, walking, talking, hearing andrepetitive motion of hands, wrists and fingers. Sedentary work withlifting requirements up to 10 pounds, sitting for extended periodsof time. Mental concentration is required in all aspects ofwork.

SALARY:
