PrideStaff is recruiting for a temporary residential Social Services Coordinator position for a senior living community located in Black Mountain .
Job Summary:
R esponsibilities include developing social assessments and care plans, coordinating admissions and discharges, and advocating for residents’ rights.
Duties and Responsibilities:
Develop social assessments and individualized care plans for each resident, incorporating input from residents, families, and friends. Coordinate the full admissions process, including waiting lists, pre-admission interviews, tours, and paperwork, communicating with all relevant parties. Coordinate discharge planning with residents, families, staff, and appropriate agency referrals. Advocate for and uphold Residents’ Rights, including documenting and following up on complaints. Provide referral and community resource information (e.g., financial assistance programs) to residents and families. Develop, implement, and maintain ongoing quality assurance programs for the social/activity department. Participate in relevant committees such as Case Management, Ethics, and Quality Assurance. Ensure compliance with the Corporate Compliance Program. Maintain confidentiality of all resident information in compliance with HIPAA.
Qualifications and Requirements:
Bachelor’s degree in Social Work or a qualified Human Services degree with at least two years of work experience with an older population preferred. Ability to function independently with flexibility, personal integrity, and effectively work with residents, personnel, and support agencies. Capacity to interact tactfully and effectively with ill, disabled, elderly, emotionally upset, and occasionally hostile individuals. Strong verbal communication skills, including fluent English reading, writing, and speaking. Possess adequate sight/hearing senses or use prosthetics to meet job requirements.
Ability to make independent decisions when necessary.
