Responsibilities Complete required documentation, including: DLA-20 Service Authorization Request Person-Centered Plan (PCP) Coordinate, access, and provide Community support services and other relevant support services in accordance with wraparound philosophy Work with a Community Support Team (CST) that operates 24/7/365 , ensuring continuous support for individuals with mental health and substance use disorders Assist individuals in community integration and ensure service intensity is adjusted as needed Maintain a beneficiary-to-staff ratio of 12:1 , with a maximum team caseload of 48 individuals What We Offer Flexible schedule Hybrid work environment Knowledge & Expertise Community Support Team policies and procedures Clinical diagnosis and treatment approaches
