Job Description
Peer Specialist 3.4 3.4 out of 5 stars Albany, NY 12206 High School diploma or equivalent and valid NYS driver’s license are required. The schedule is 12 hours/week, Monday-Friday, hours are flexible. As per OMH Guidelines, the candidate must have lived experience with homelessness and/or mental illness. The RSS Albany Transition Support Team has an opening for a Peer Specialist. The peer works with individuals transitioning out of the Capital District Psychiatric Center, as well as individuals within the community to provide intensive support for community integration and stability within their apartments. The Peer Specialist will play an active role in assisting clients with preventing avoidable use of crisis services, the ER and hospitalization. The Peer Specialist will act as a support and advocate for clients by providing insight from their own lived experience (when appropriate). The Peer Specialist will act as a liaison to the Transition Support Team and ensure that individuals are following their created safety plan and following through on established goals to avoid returning to crisis or the hospital.
Main responsibilities include:
providing ongoing support, assistance with appointments, referrals to services, transportation to assist with linking to services, assistance with benefits, and WRAP planning. The selected candidate will also assist people with integrating into the community, including identifying opportunities/resources for employment, education, peer supports, hobbies, involvement in community activities, use of public transportation, and more. The candidate should have a positive attitude; feel comfortable navigating services in the community, and exhibit professionalism. #HP123
