The Screener Navigator is responsible for screening Medicaid Members for their social care needs; conducting social care navigation, including eligibility assessment and referral to supportive programs; and providing certain enhanced services to meet individuals’ needs. his role advances DWDC’s mission by linking underserved individuals with the services and support that address their unique needs. Using a standardized assessment tool, the Screener Navigator identifies members’ social care needs, determines eligibility, and ensures access to suitable resources. Members may be directed to Social Care Network programs or other local, state, and federal assistance services, depending on their specific needs. Additionally, the Screener Navigator will collaborate with internal staff and partner agencies within the Social Care Network to streamline support efforts and enhance service delivery. Duties Conduct screenings to evaluate individuals’ needs and determine eligibility for services. Utilize crisis management techniques to effectively address urgent situations. Conduct social care navigation to connect participants to appropriate services. Collaborate with DWDC’s staff as well as local, city and state partners to advance DWDC’s mission and address participants’ unique needs. Maintain accurate records of assessments and follow-up actions in compliance with SOMOS-SCN and organizational policies. Experience High school diploma or GED, plus at least two years’ experience in social services; Bachelor’s Degree in Health and Human Services or related field. One year experience in eligibility screening, social services, or healthcare preferred or Community Health Worker. Ability to relate to and work with underserved and vulnerable communities. Strong interpersonal and communication skills are essential for building rapport and trust with individuals Integrity, positive attitude, mission-driven, and self-directed with demonstrated passion for DWDC’s mission. Detail-oriented and have competent literacy and writing, documentation, communications and interpersonal abilities (friendly, courteous, helpful, ability to work as part of a team).
