EVP of Programs Job Summary The Urban League of Rochester seeks a Full-Time Outreach Specialist, who will play a critical role in ensuring the League achieves its plans for growth in alignment with its mission and vision. This individual will report to the Executive Vice President of Programs and will be responsible for the day-to-day support and service delivery of our Family Services Division . This position provides an outstanding opportunity for a team member to make their mark at a reputable, growing non- profit. Candidates must possess a passion for community will an ability to travel in order to connect with potential partners and clients seeking information related to services provided. This position will act as liaison – creating space for both ULR and EPI to continue to work together. Responsibilities and Duties Work within community to meet families and advise them of the resources available Recruit for Day Hab Collect data and report with efficiency and accuracy the finding based on the surveys and observation Attend community events, agency fairs, school events, attend RCSD Special Education meeting and RCSD Parent Engagement meetings to share information and resources to families and network with other entitles serving Monroe County communities. Increase families general understanding of I/DD Provide information on types of services available including the Office for People with Developmental Disabilities (OPWDD) eligibility and other critical support systems and the benefits these services can have. Continue conversation from initial contact to educate, empower, and provide guidance on available resources, programs, and opportunities to enhance family and individual quality of life Assesses family and individual needs Provides specific recommendations of services available based on those needs. Explains process and requirements for applying for OPWDD services or contacting/applying for programs if already OPWDD eligible. Enroll families into the program, devise an overall plan with the family for the services and/or the eligibility they are looking to acquire. Service provided in manner determined by family – phone call, in-person, or virtual meeting. In-person meeting locations determined by the person/family. We will meet them where they are at (in their homes, out in the community, in-office, etc.). Executes the plan created in the navigation phase. Hands-on assistance with referrals to include assisting with gathering documentation needed for eligibility, working with care managers, other necessary responsibilities as determined on a case by case basis. Assist with starting the process for OPWDD eligibility, when necessary, and/or other needed and desired services. If already OPWDD eligible, will assist with contacting and applying for programs via care manager involvement Provide follow along support via monthly contact for 12 months following official enrollment into program.
Requirements:
REQUIRED
Associate’s degree in Human Services, Public Administration, Communications, or a related major (Bachelor’s degree in Human Services).
PREFERRED
