The Housing Specialist motivates and supports individual patients to achieve their goals, through screening for social determinants of health and implementing necessary interventions.

DO NOTE THAT THIS POSITION IS TEMPORARY/GRANT FUNDED
- Engage Bridge to Home guests and support them to identify their health and housing goals.
- Meet with guest onsite and throughout the building
- Conduct screenings and interviews to identify and prioritize their health and social services goals
- Support and motivate guest to accomplish their stated goals
- Assist patients to complete a supportive housing application
- Facilitate referral to Housing for Health team for housing placement
- Educate guests regarding available community services and facilitate warm hand off to community-based supports
- Educate guests regarding available community services, health services, and rights
- Assist guests in obtaining all necessary documentation to facilitate benefits application process
- Document each encounter in the electronic medical record (Epic) and use other electronic systems (e.g. FindHelp or other HIPAA compliant patient portals) in accordance with established policies and procedures.
- Attend regular team meetings to discuss guests’ progress and update care plans; communicate with guests’ providers
- Participate in ongoing education to learn and maintain skills
- Perform other, related program tasks, as needed and perform duties in response to public health emergencies, when necessary.
Other duties as assigned Experience . Two additional years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or 2. Education and/or experience equivalent to “1” above. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above. However, all persons must have at least one year of the full-time experience described above Knowledge of Section 8 housing programs and LIHTC regulations is beneficial. Strong negotiation skills to facilitate agreements between clients and landlords. Understanding of Fair Housing regulations is essential for compliance purposes. Proficiency in using property management software such as Yardi is advantageous.
