As a Community Liaison, you’ll identify and establish relationships with referral sources. Your goals will be to maintain and grow key partnerships, increase revenue, and help prospective clients connect with the care they need. You will also spend designated hours each week following up on client leads and may at times assist with in-home visits to support client assessments and service coordination. In the future, you may also supervise and collaborate with an Associate Community Liaison in your territory, who will support lead follow-up and client service coordination to support your business. This associate will have a dotted-line reporting relationship to the Community Liaison. This is a key role with the potential to significantly grow our business in the community. You’ll have the support, resources, and trust of leadership to make a real impact. This role offers uncapped bonus potential based on the business you drive, plus a base salary typically between $60,000 and $70,000. Higher base salaries may be considered for exceptional candidates with proven success in home care or referral-based sales.   • You’ll have weekly check-ins with the President to review performance, align on strategy, and identify opportunities to elevate the agency’s impact.
  • You’ll also connect regularly with our office staff to share feedback, discuss lead quality, coordinate care transitions, and improve how we operate as a team.

What You’ll Do Build and maintain relationships with professionals and community organizations — including hospitals, skilled nursing facilities, assisted living communities, hospices, social workers, and others — who may refer individuals in need of home care services Maintain a database of contacts, track sales efforts, and report on activities and results Conduct presentations, face-to-face meetings, and phone calls with prospective and current referral partners to share information about FirstLight Home Care Participate in community networking groups, especially those related to the senior population Spend a set number of hours each week following up on incoming client leads and working to convert them into in-home consultations and clients Assist with in-home visits to help assess client needs and support the care coordination team Collaborate with the care coordinator and, in the future, potentially with an Associate Community Liaison to ensure timely lead follow-up and high-quality client service Desired Skills and Qualifications You are genuine, relatable, and trustworthy, with a natural ability to connect with referral sources and community partners You understand the difference between building a relationship and asking for the business — and you’re confident doing both You take a strategic, data-informed approach to business development, focusing your time where it matters most You’re intentional and judicious with how you engage (this is not about merely dropping off materials), but build trust and deliver clear, valuable messages that are in line with business focus You document interactions and maintain CRM notes because you know great records are the foundation of great results You prepare in advance — practicing thoughtful pre-call planning and making every touchpoint count You’re coachable, open to feedback, and committed to learning and growing You’re an extraordinary team player who understands how sales and operations must work together for long-term client satisfaction You’re curious about operations and eager to learn how to ensure your sales translate into excellent care experiences You’re willing to jump in and help wherever needed — because teamwork makes us all better.
