Position Summary:

As our Community Liaison , you’ll be the face of Synergy HomeCare in the community—building relationships with hospitals, physicians, rehabilitation centers, assisted living facilities, hospices, and more. Your focus will be on identifying potential clients and referral sources, while educating the public about the benefits of home care. This is a relationship-building, mission-driven role for someone who thrives in outreach, marketing, and community engagement.

Core Responsibilities:

Community Engagement & Outreach o Develop and maintain strong, strategic relationships with: o Hospitals and discharge planners o Physicians and medical offices o Skilled nursing and rehabilitation centers o Assisted Living Facilities and Memory Care communities o Hospice providers o Represent Synergy HomeCare at health fairs, networking events, and community meetings. o Organize and deliver educational presentations to promote awareness of in-home care services and resources. o Promote the benefits of home care to both professional referral sources and the general public. Sales & Business Development Conduct outside sales calls within an assigned territory. Secure referrals and build a consistent pipeline of new clients. Act as the exclusive point of contact for special programs like VA program and the VCC Aide & Attendance benefits . Promote Synergy HomeCare programs to medical professionals, civic groups, and community organizations. Track outreach activity, referral data, and lead conversions.

Qualifications:

1-Year previous experience in home care , hospice , healthcare marketing , or senior living (required) Strong knowledge of the healthcare landscape and referral process Exceptional communication , networking , and relationship-building skills Passion for serving the community and families in need of care Self-motivated with a goal-oriented mindset Valid driver’s license and reliable transportation

Valid Auto Insurance Compensation & Perks:
