to join our rapidly growing team as an Assessment Counselor. The Assessment Counselor is responsible for providing group and individual counseling to program clients. The Assessment Counselor will complete comprehensive assessments for patients presenting for services with substance use disorders. The Assessment Counselor Is Expected To Responsibilities and Duties Assess program clients and participate in a comprehensive evaluation of each client to screen for substance abuse history/issues. Document, implement and update the treatment and/or individual service plan for clients as assigned. Knowledge of

regulations and documentation requirements. Write Daily progress notes, maintain all documentation requirements in a timely manner. Identify and develop relationships with community based providers. Background in Case Management a plus – strong knowledge of community resources. Perform other duties as may be assigned. Qualifications And Skills Valid New York State CASAC-T, CASAC, MSW or LMSW credential required. High School Diploma required, BA/BS preferred. Two years’ experience working with clients with history of alcohol or substance abuse preferred.
