PHP IOP primary counselor 3.5 3.5 out of 5 stars Olyphant, PA 18447 The PHP/IOP Substance Use Disorder Counselor is responsible for providing clinical services to individuals enrolled in Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) levels of care. This includes conducting assessments, developing individualized treatment plans, facilitating group and individual therapy, documenting services accurately, and collaborating with the interdisciplinary team to ensure comprehensive care.

Essential Duties and Responsibilities:

Conduct biopsychosocial assessments and collaborate in determining appropriate level of care. Develop and maintain individualized treatment plans in accordance with ASAM criteria. Facilitate evidence-based individual and group counseling sessions targeting substance use and co-occurring disorders. Maintain accurate and timely clinical documentation including progress notes, treatment plans, treatment plan updates, discharge summaries, etc. Provide crisis intervention and support as needed. Participate in clinical staffing, supervision, and case reviews. Coordinate care with referral sources, family members, medical providers, and other professionals as appropriate. Ensure services are compliant with state regulations, licensing bodies, and payer requirements. Promote a client-centered, trauma-informed approach in all clinical services.
