SUMMARY:

The Success Coach/Counselor’s role is to provide essential support to the participating youth in Camp Bravery’s Children Services Council program by advising, coaching, mentoring, encouraging and to advocate in helping guide youth on a successful, positive and educational path. Identify and assess youth’s strengths and challenges in order to coordinate, encourage and assist in ensuring the necessary services are obtained for the youth. Actively work with youth to identify and attain individual goals. This position includes performing recreational activities and administrative functions for various youth age groups in the program.

DUTIES AND RESPONSIBILITIES

Develops and maintains daily activity schedule and calendar of events and special topics for weekly classes with the team. Facilitates services related to the program activities according to calendar of events. Leads group activities to promote positive peer relationship. Keeps track of weekly activities, maintaining daily logs with client signatures for accountability of hours worked and enter units of service into billing logs on a weekly basis so invoicing can be prepared in a timely manner. Creates, reviews and update youth’s Individual Service Plan, Education Update form in accordance with contract requirements and monitor youth’s progress /challenges in completing goals. Conducts mock interviews with youth interviewing for scholarships and/or employment opportunities. Works with Team Lead to coordinate and facilitate mentor-youth activities to maximize mentor involvement with youth. Implements youth’s educational plan, follow up on youth’s educational challenges and arrange meetings with youth who are performing poorly in school in order to assess and address key issues and develop an action plan to increase academic development. Assesses the needs for internal and external services and community resources and makes appropriate referrals, including following up with other agencies, caseworkers, and the family to assure needs have been met. Provides case management and informal counseling to youth and caregivers regularly and during times of crisis in order to assist in identifying and planning resolutions to conflicts. Addresses concerns of and maintain open/collaborative communication with referral sources, mentors, caregivers, caseworkers, etc. regarding referrals, youth compliance, services given/received and youth progress toward goals. Keeps accurate information regarding youth class attendance and program participation including group notes, copies of handouts/class material and sign-in sheets. Assures all group notes are turned in on time and appropriate paperwork is filed in youth’s binder. Evaluates and assesses the needs and strengths of clients and families. Administers outcomes and assessments thoroughly and in a timely manner in accordance with contract requirements. Enter chronological notes into youth’s computerized file for units billed and services provided. Notes must be updated weekly and cannot be more than 1 week behind at any given time. Compliance will be checked during supervision a minimum of twice per month. Conducts case management, school visits as well as any other field work needed to stabilize or support youth in reaching goals. Transport youth as necessary for various activities including but not limited to program activities and events. Must maintain confidentiality, moral codes of conduct and professionalism at all times. Carries a caseload of up to 25 to 30 youths to provide well-rounded case management services in efforts to fulfill the needs of our disadvantaged youth. Actively participates in agency activities to include weekend. Require to attend all planned trainings and staff meetings. Enforces safety policies and procedures. Assists in the rendering of first aid in cases of minor injuries. Maintains records and prepares reports as directed. Performs other related duties as assigned by management.

SUPERVISORY RESPONSIBILITIES

This job has no supervisory responsibilities.

QUALIFICATIONS

Bachelor’s Degree (BA) from four-year college or university, or one to two years of related experience and/or training, or equivalent combination of education and experience. Certificates, licenses and registrations required: Valid Florida Driver’s Licensing Must be able to pass a Level 2 ACHA background screening.

Computer skills required:

Development Software; Microsoft Office Suite Other skills required: Previous experience in a similar environment preferred. Knowledge and understanding of the interests and characteristics of various age groups, and the ability to apply this knowledge in supervision of and participation in activities. Knowledge of the principles, practices and techniques of public educational and recreational activities and programs, and of major instructional procedures, rules and regulations. Knowledge of basic safety and first aid procedures. Ability to fully administer and teach the assigned curriculum. Ability to understand and follow oral and written instructions. Ability to keep records and prepare reports. Ability to organize and actively participate in various indoor and outdoor educational and recreational activities. Ability to serve the public and fellow employees with honesty and integrity in full accord with the letter and spirit of Broward County’s Ethics and Conflict of Interest policies. Ability to establish and maintain effective working relationships with the general public, co-workers, elected and appointed officials and members of diverse cultural and linguistic backgrounds regardless of race, religion, age, sex, disability or political affiliation.
