We are seeking a dedicated, organized, and student-focused individual to serve as a School-Based Team (SBT) Member for the upcoming school year.
Job Description:
The School-Based Team (SBT) Member will play a key role in identifying, planning, implementing, and monitoring intervention strategies to support students who are struggling academically, behaviorally, socially, or emotionally. As part of a collaborative, multidisciplinary team, the SBT Member will work closely with teachers, administrators, counselors, and families to ensure students receive the support they need to succeed in a K-12 environment.
Key Responsibilities:
Participate in regular SBT meetings to review student referrals and concerns. Collaborate with teachers, school counselors, ESE staff, and administration to develop and implement academic and behavioral intervention plans. Monitor student progress and document intervention effectiveness through data collection and review. Assist in identifying students who may require further evaluation for Exceptional Student Education (ESE), 504 Plans, or other support services. Communicate regularly with families regarding student progress and next steps. Ensure interventions are developmentally appropriate and culturally responsive across all grade levels (K-12). Maintain accurate records in compliance with school and district policies. Support and promote a school-wide culture of proactive, data-driven student support.
