Join Easterseals PORT Health (ESPH) as a Family Partner on our Wraparound Services team. In this impactful role, you’ll use your own experience to empower families, guide them through complex systems, and support them in becoming strong advocates for their children. As an essential member of the Child and Family Team (CFT), you’ll help shape care that is family-driven, culturally responsive, and grounded in hope and resilience. This is more than a job, it’s a chance to turn your personal journey into powerful support for families facing similar challenges. What You’ll Do The Family Partner uses lived experience raising a child with behavioral health needs to provide one-on-one, strengths-based support to families. As a key member of the Child and Family Team (CFT), the Family Partner empowers caregivers to advocate for themselves, navigate services, and build resiliency. This role involves mentoring families, supporting care coordination, connecting them to community resources, and promoting positive outcomes through collaborative, family-driven planning.
