Full-Time SUMMARY:
The primary role of the In Reach Coordinator is to screen and liaison with the court and correctional systems on referrals to the Mental Health Offenders Program (MHOP) or other Sulzbacher programs. They will assess individuals with substance abuse and/or co-occurring disorders. The role of the In Reach Coordinator is important in coordinating required and needed services from across service systems (i.e., mental health, criminal justice, developmental services, and addictions). This person will work in the Jail, facilitating screening of incarcerated individuals, attending court with them, coordinating with court, JSO, public defender and state attorney representatives to facilitate program entry and ongoing inclusion. This position fulfills a vital function for persons served by working with them to realize personal recovery goals. . .

ESSENTIAL DUTIES AND RESPONSIBILITIES
: Interact directly with clients in a correctional setting Provide accurate information about available assistance and complete necessary referrals Attend Duval County Court Coordinate the application process for substance use offenders appearing in Court Help educate/inform judges, attorneys, and other court personnel about addiction, recovery and the treatment process Obtain appropriate access and proficiently use correctional software to coordinate referrals Report acceptance/rejection of referrals to supervisor and Court in a timely fashion Communicate daily with the MHOP Court Coordinator and MHOP Manager to maintain a current list of inmates in, accepted to, and awaiting admission to MHOP Enter all data into Athena (EHR) and billing system(s). Maintain thorough and complete documentation of all services as required by state regulation, accreditation standards, Sulzbacher policy and all legal requirements Work collaboratively with the team to complete required reports in order to document program performance outcome measures in Excel Work closely with the closely with the court, Correctional system, Correctional Healthcare system, and affiliated stakeholders in a positive and helpful manner Attend staff development trainings and meetings as directed Participation in weekend coverage schedule Cross training with other roles on the MHOP team All other duties as assigned

The In Reach Coordinator is sensitive to emotional, spiritual and practical needs of persons served, guests and staff members in all interactions. Provides positive feedback and comments about the quality of work and effort that has been offered by persons served and coworkers. Adjusts priorities and schedule to meet specific persons served needs. Reports and/or corrects problems related to safety, cleanliness and comfort of persons served and work areas. Universal Precautions and Infection Control techniques are followed. Understands their own role in facility safety plans and procedures related to fire, weather and other disasters.
EDUCATION AND EXPERIENCE
A bachelor’s degree in human services preferred Able to work a flexible schedule 2 years’ experience working in the mental health or human services field Ability to establish cooperative working relationships with persons served, colleagues, community partners, and volunteers Ability to interact with people from diverse cultural and racial backgrounds Excellent customer service skills Excellent interpersonal skills, leadership skills, and ability to work independently in a fast-paced environment Ability to manage and de-escalate crisis situations Working knowledge of community resources Good computer skills needed Valid Driver’s License and ability to travel Acceptable annual Motor Vehicle Record check Adhere to required insurance coverage levels (
