JOB DEFINITION
Work with the CN Guidance Forensic ACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches.
EDUCATIONAL REQUIREMENTS
A bachelor’s degree in a related field.
EXPERIENCE REQUIRED
Two years’ experience working with the chronically mentally ill. Competency in substance abuse, family support, or vocational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment. Two years of experience working with formally incarcerated clients and/or working with justice involved clients. Knowledge of violence risk assessments and court procedures are a plus.
DUTIES AND RESPONSIBILITIES
1. Perform routine team duties, and have lead responsibility for justice involved clients with regards to integrating dual-recovery treatment, family support or vocational/employment training with the tasks of other team members. 2. Has lead responsibility for navigating the criminal justice system, advocating on behalf of FACT individuals, and providing education and skill building. 3. The Criminal Justice Liaison is the supporting liaison with key criminal justice system stakeholders and conducts in-reach to people who are incarcerated, including following initial referral, to gather data concerning the individual’s criminal history. 4. The Criminal Justice Liaison will spearhead jail release planning to provide immediate support for the transition back to the community. 5. The Criminal Justice Liaison is expected to be familiar with county-specific ATI programs 6. Complete any required AOT, parole, probation, or court progress reports and escort consumers to related hearings and appointments 7. Involve family, significant others, housing providers, defense attorney, and/or community corrections to ensures compliance with regulations and coordination of services. 8. Assist the ACT team and consumer in the development of service plan goals and objectives that promote reductions in criminogenic risk and prosocial community living 9. Coordinate with jail and prison reentry coordinators and visit consumers during detention to facilitate their safe and successful reenter 10. Coordinate, advocate and liaise with probation and parole officers 11. Responsible for providing Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between Forensic ACT and other systems. 12. Engage in performing assessments, case management, handling crisis intervention, and discharge planning 13. Maintains case records and completing documentation on time and in a matter in compliance with federal, state and agency regulations. 14. Provides direct, face-to-face and/or collateral services to recipients either in the Forensic ACT Office or in the community in places and situations where problems arise. 15. Responsible for periodic on call coverage to be arranged in advance with the Program Director or Supervisor as needed 16. Assists recipients in developing appropriate expectations from treatment. 17. Strengthens the ability of the recipient to understand and utilize therapy and find resources after treatment has ended through proper discharge planning and termination process. 18. Utilizes and understand the value of supervision, which increases practice skills, promotes better services for the recipient and preserves accountability to the Agency. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress 19. Complete QI Training during onboarding and participate in QI refresher training as needed. 20.
