The Director of Patient Safety and Quality providesleadership, direction and administration to ensure consistency,follow through and successful completion of established strategicobjectives. The director will focus on developing cross-functionaland system-wide relationships beyond the Department of PatientSafety and Quality to facilitate highly multidisciplinary andcomplex collaborations. Quality goals across the assigned focusedareas are monitored, barriers eliminated and are successfullyachieved. The director will facilitate the transformation ofoperations by instituting and nurturing an outstanding model forcollaboration and teamwork, safety, and high quality, reliableoutcomes. The ideal candidate will be well-versed in data analysis,quality improvement methodologies, patient safety principles, casereviews, and using Epic. Familiarity with dashboards, maternalhealth metrics, and publicly reported metrics is also essential.

This posiiton will offer a hybrid schedule. Candidates must havethe ability to travel to various hospital sites within the network.

Responsibilities A day in the life of a Director of Patient Safetyand Quality at Hackensack Meridian Health includes:

  • Partners withVP of Patient Safety and High Reliability and other departmentleaders to provide vision, leadership, structure and direction forcontinued development of the Department of Patient Safety andQuality.
  • Executes the Department of Patient Safety and Qualitystrategic plan across the network including planning and directingsafety and quality.
  • Promotes a culture of patient safety andleads systemic implementation of an effective patient safety andquality improvement program.
  • Facilitates physician and nurseleaders, and staff toward process and system improvements that willsupport the reduction of preventable healthcare errors and otherfactors that contribute to unintended adverse events and/or qualityoutcomes.
  • Lead a prioritization and action planning process thatfocuses on eliminating health disparities.
  • Collaborate with DataAnalytics and Business Intelligence leadership collecting relevantdata for quality improvement and research projects.
  • Attends,facilitates relevant HMH meetings for quality and safety.
  • Appliesto Hospital based positions only- Focus on key areas including butnot limited to women/maternal health, perinatal/infant and socialdeterminants of health; in concert with OB leaders across HMH,serve as the women/maternal health, perinatal/infant health PatientSafety and Quality content expert by designing, planning,implementing and coordinating safety, quality and performanceimprovement activities for all birthing hospitals (and outpatientfacilities).
  • Oversees and facilitates Root Case Analysis (RCA)and Apparent Cause Analysis (ACA) process and evaluations.
  • Maintains up to date knowledge for the practice of patient safety,performance improvement, and high reliability: Integratesprinciples using the theories, methods, tools and principles ofpatient safety, performance improvement, and high reliability,including statistical data collection and analysis.
  • Stays abreastof and communicates all regulatory compliance changes andincorporates findings into all resident/patient safety and qualityimprovement activities accordingly.
  • Foster continuousorganizational learning and scaling best practices across thenetwork and between community partners through sharing informationand lessons learned electronically, via workshops, tool kits etc.
  • Develop and improve meaningful patient outcome metrics throughcollaboration with front-line teams, leadership and the SteeringCommittee. Ensure the implementation of best evidence-basedpractices.
  • Promote external alliances by:
  • dentifying andbuilding relationships with strategic institutional partners
  • leveraging existing and building new alliances with community-basedhealth organizations
  • developing and maintaining strongrelationships with local government agencies.
  • Strategize withleaders to lobby policymakers, track legislative and regulatorypriorities, produce advocacy materials, and drive meaningful changefor better outcomes.
  • Applies to Hospital based positions only-Participates in physician recruitment and physician practicecontracts focusing on improved maternal -fetal health in mind.
  • Other duties and/or projects as assigned.
  • Adheres to HMHOrganizational competencies and standards of behavior.
Qualifications Education, Knowledge, Skills and Abilities Required:
  • Masters Degree in Nursing, Health Care Administration, PublicHealth, or other advanced health-related degree
  • 7-10 years ofclinical experience in an acute care hospital or relevant HealthCare experience
  • Applies to Hospital based positionsonly-Experience with NDNQI & Magnet Accreditation.
  • 3-5 years ofexperience in patient safety and quality.
  • Strong communicationand presentation skills.
  • Experience in the use of computerapplications and software.
  • Excellent written and oralcommunication skills.
Education, Knowledge, Skills and AbilitiesPreferred:
  • Performance Improvement expertise.
  • HRO experience.
  • Mastery of performance improvement methodologies.
  • Highlycollaborative leader.
  • Attainment of CPPS (Certified ProfessionalPatient Safety) within one year of hire.
Licenses andCertifications Required:
  • Certified Professional in HealthcareQuality or Certified Healthcare Quality Management or QualityManagement Certification, or other related National Qualitycertification.
