SUMMARY OF POSITION
The Discharge Support Specialist serves as a key point of support during patient discharge, providing the final opportunity to engage individuals at risk of leaving treatment Against Medical Advice (AMA) or Against Clinical Advice (ACA). This role requires the ability to build positive rapport, promote continued care, and ensure a respectful discharge process. The position also supports admissions and intake functions, including screening patients, verifying insurance, and monitoring individuals during the intake process to ensure safety and compliance. Strong communication, empathy, and professionalism are essential.
PRIMARY RESPONSIBILITIES
Accompany and transport discharged patients to designated dropoff points (Wollaston or Quincy Center train stations) via walking or agency vehicle Essential to accompany and transport discharged patients to designated dropoff points, regardless of weather conditions. Coordinate with the Operations Manager to allocate and issue MBTA transportation passes to eligible patients. Accurately document all discharge activity using the McGahan Center Discharge Log. Maintain and update the Van Log to ensure proper tracking and compliance. Provide compassionate and respectful guidance to discharged individuals, demonstrating patience and empathy throughout the process. Establish and uphold professional boundaries while fostering a supportive and respectful environment. Communicate effectively with patients and staff, employing strong interpersonal and deescalation skills when necessary. Respond to emergencies by executing appropriate code protocol procedures in a timely and composed manner. Manage multiple tasks efficiently and adapt to shifting priorities in a dynamic, fastpaced setting.
ADMISSIONS & INTAKE FUNCTIONS
Conduct initial patient screenings, including insurance verification and bed assignment coordination. Oversee and monitor patients in the waiting area to ensure safety and adherence to intake procedures. Perform property searches, log personal belongings and valuables, and check for prohibited items in accordance with agency policy. Operate the bed bug treatment equipment as part of the intake sanitation protocol.
REQUIREMENTS
Minimum of two (2) years of experience in the substance use treatment field; experience working with individuals with cooccurring behavioral health disorders is strongly preferred. Recovery Coach Certification is preferred but not required. Must possess a valid Massachusetts driver’s license and be willing to submit a driving record for review. Must consent to and successfully complete a preemployment background check, including CORI and drug screening. Strong understanding of, and adherence to, federal and state confidentiality regulations, including 42 CFR Part 2 and HIPAA. Proven ability to maintain confidentiality, demonstrate professionalism, and uphold clear personal and professional boundaries. Individuals with lived experience are encouraged to apply; candidates in recovery must have a minimum of two (2) years of continuous sobriety. Must exhibit key interpersonal qualities such as empathy, dedication, authenticity, and cultural sensitivity. Excellent verbal and written communication skills, with the ability to interact effectively with diverse populations.
