Job Summary:

The Patient SafetySpecialist (PSS) will be responsible for planning, organizing, andfacilitating programs to ensure that all clinical departments arefollowing the Joint Commission’s National Patient Safety Goals(NPSG’s) and MWHC’s patient safety program. The PSS will collectdata through chart reviews and other methods while working withmembers of MWHC to identify opportunities to drive improvements inpatient safety clinical outcomes and practice variation.

EssentialFunctions & Responsibilities:

Assists with the identification ofstrategies to create and reinforce a culture of safety andcontinuous preparedness for NPSGs. Collects clinical data andconducts chart reviews through utilization of the hospital medicalrecord and information systems. Supports a culture for patientsafety by communicating with nursing, administration, and medicalstaff leadership to identify ways to ensure organizationalcompliance with all regulations and policies. Drives patient safetyimprovements by identifying trends, using significant outcomesperformance data and recommends opportunities to enhance clinicaloutcomes. Communicates patient safety actions and recommendationsto committees, department directors, and employees. Coordinates anyongoing patient safety orientation, activities, and continuingeducation programs for staff members. Leads projects which willimpact patient safety and support the strategic goals of theorganization using a variety of process improvement conceptspractices and procedures. Leads investigations for serious safetyevents using FMEA and/or Root Cause Analysis methodologies;develops action plans and ensures implementation of identifiedimprovements. Prepares annual report of actual error occurrencesand actions taken to improve patient safety. Manages error/eventreporting through a non-punitive error reporting system. Performsother duties as assigned.

Qualifications:

Graduate of an accreditednursing program required; BSN preferred. Valid RN License fromVirginia in good standing or hold a current multistate licensureprivilege as an RN required. Minimum of three years of clinicalexperience required.
