• Position Highlights:

• + This is a Hybrid role, which will include community palliative & hospice and mixed locations (patient homes, Acute, SNFs, and Office) +

Geographic Focus:

Mecklenburg County & Cabarrus County +

Full-Time Schedule:

Monday – Friday, 8AM – 5PM +

Call Schedule:

1 in 4 (with APP taking 1 st call) + Join 4 Physicians & 6 APPs + Strong referral base + Program hosts medical students, residents, APP and MD fellowships + Chaplain support is available

• Ideal Candidate:
