Summary:
The Community Relations Specialist is responsible for generating qualified referrals by identifying, engaging, and cultivating relationships with physicians, healthcare facilities, and community partners in the home health and hospice market. This role serves as a primary outreach representative, educating referral sources about the organization’s services and value proposition. Community Relations Specialist works in partnership with the Clinical Liaison to ensure seamless communication and continuity of care. Success in this role is driven by the ability to build trust, foster long-term partnerships, and consistently drive new referral opportunities. Qualification Requirements Bachelor’s Degree or equivalent required OR Associate’s Degree and a level of experience that surpasses current qualifications 3+ years of experience in business development, sales, or partnerships within the healthcare sector Familiarity with the U.S. healthcare landscape, including knowledge of payors, providers, value-based care, and health IT Excellent communication skills, both written and oral, and the ability to demonstrate successful negotiation skills. Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors. Responsible for market outreach and referral generation. Identify new referral prospects including physicians, hospitals, SNFs, ALFs, clinics, and community health organizations. Conduct regular field visits, presentations, and in-service meetings to educate referral sources on home health and hospice services. Develop and maintain a prospect pipeline, aligned with territory growth goals. Responsible for relationship building and strengthening professional relationships with key stakeholders, including discharge planners, case managers, practice administrators, and physician liaisons. Maintain ongoing communication and presence with current and prospective referral sources to increase engagement and loyalty. Respond to referral inquiries and provide updates on patient admissions or other feedback as needed. Track and manage outreach campaigns, events, and other initiatives. Support the Clinical Liaison in maintaining strong connections with referral sources and other community partners. Routinely track and report referral activity, visit frequency, and outcomes using Gridlex CRM. Maintain up-to-date and accurate documentation in Gridlex CRM. Responsible for attending marketing meeting, huddles, and other internal collaborations unless excused by the Marketing Director. Demonstrates an interest in personal and professional growth: (a) attends and participates in workshops, seminars, webcasts to keep abreast of current changes in rules, regulations, relating to job functions; (b) does individual reading and research. Demonstrates a willingness to be cost effective in the use of agency resources, the monitoring of waste, and the proper and safe use of supplies and equipment.
