Position Summary:
The Community Liaison plays a vital role in expanding patient referrals and building relationships with physicians, hospitals, and community agencies. This individual will promote the nurse registry’s services, coordinate referral activities, and represent the organization in the community with professionalism and compassion.
Key Responsibilities:
Referral Development & Relationship Management Cultivate and maintain strong relationships with healthcare providers and referral sources. Conduct routine visits to hospitals, physician practices, skilled nursing facilities, and community organizations. Serve as the primary point of contact for referral partners, addressing questions and facilitating smooth care transitions. Outreach & Marketing Promote awareness of nurse registry services through direct outreach and marketing campaigns. Attend and represent the organization at community events, health fairs, and professional networking functions. Maintain a schedule of outreach activities and manage distribution of promotional materials. Patient Intake Coordination Assist referral sources and clients in understanding services and initiating care. Coordinate closely with the intake and scheduling teams to ensure timely patient onboarding. Data Tracking & Reporting Monitor referral trends and outreach effectiveness using CRM or internal tracking tools. Provide monthly reports on referral volume, source activity, and growth opportunities. Compliance & Ethics Adhere to HIPAA regulations and the nurse registry’s standards for ethical marketing and referral practices. Maintain confidentiality and represent the organization’s mission and values in the community.
Required Qualifications:
Education:
Associate’s or Bachelor’s degree in Marketing, Healthcare Administration, Business, Communications , or a related field preferred.
Experience:
Case management or patient referral coordination . Experience working with home health agencies, nurse registries, hospitals , or similar healthcare providers is strongly preferred.
Skills & Competencies:
Strong interpersonal and communication skills Knowledge of local healthcare systems and referral pathways Self-motivated with excellent time management Ability to travel locally for visits and events (must have reliable transportation) Proficient in Microsoft Office and CRM tools (preferred) Preferred but
Not Required:
Clinical background (LPN, RN, or CNA certification) Familiarity with
HIPAA, CMS
guidelines, and nurse registry operations
Job Types:

$22.65 – $27.27 per hour Expected hours: 24 per week
