Essential Responsibilities and Duties:

Other Duties May Be Assigned) Initiate and maintain healthcare contacts within the assigned territory. Maintain contact with hospitals, physicians, physician’s staff, discharge planners, social workers. Serve as a liaison between patients, physicians and agency staff to ensure that the patient’s needs are being met. Pre-screen patients from physicians, hospitals, long-term care facilities and assisted living facilities for home health needs, eligibility and status, when referral is concurrent with discharge. Make marketing presentations to current and prospective clients to educate them on the agency’s services. Deliver orders to physician’s offices for signature and ensure timely return of the orders. Communicate with each referral source to ensure that the expectations of the referral sources and the needs of the patient are being met. Provide timely, accurate completion of reports, worksheets, and information as requested by the Administrator/designee. Participate in company-sponsored programs and meetings and maintain a networking base in the community through trade associations, business organizations and local association meetings. Maintain knowledge of the agency’s policies and procedures and Medicare regulations that are applicable to home health care. Able to work cooperatively as a member of a team.

Qualifications:

Experience:

2+ years of sales experience in home care and/or health-related field preferred

Language Skills:

Must possess good communication skills, both written and verbal in English. Ability to communicate in other foreign languages is desirable.

Mathematical Skills:

Math skills required

Reasoning Ability:

Able to work under pressure and handle multi-task functions. Must be able to deal effectively with people. Must be self-motivated and resourceful.
