Position Summary:
The Client Relations Manager plays a critical role in ensuring outstanding client experience. This position serves as the primary point of contact for our clients and families, supporting them before, during, and after-care is provided. You will handle welcome calls, follow-ups, conflict resolution, hospital outreach, client scheduling, and more. A background in social work is highly preferred.

- Regular office hours Monday – Friday 8am-5pm.

Bonus Structure Key Responsibilities:
- Conduct welcome calls with families before their first shift and follow up after the initial visit to ensure satisfaction.
- Serve as the primary point of contact for client and/or family concerns and conflicts, resolving issues promptly and empathetically.
- Make routine calls to family members or designated contacts to check on clients during hospital stays and assist with post-discharge care planning.
- Manage the creation of CHOICES client profiles in the company CRM and ensure authorizations are created and billed accurately.
- Oversee all client activations and deactivations in the scheduling software to ensure accurate service coordination.
- Update client care plans as needed to reflect changes in medical conditions.
- Conduct monthly outreach to each client to assess service satisfaction and address any concerns.
- Participate in weekly new client meetings, providing updates on each new client and reporting on how services are progressing.
- Participate in state-reportable events and address matters involving adult protective services and other client-related issues.
- Collaborate daily with the scheduling, business development, and quality care manager team to ensure smooth service delivery.
- Actively participate in daily team meetings.
- Maintain accurate, detailed documentation in all client records.

Qualifications:
- Bachelor’s degree in social work or a related field (preferred).
- Strong interpersonal skills with a compassionate, patient, and people-focused approach.
- Excellent organizational skills and attention to detail.
- Proficient in Microsoft Word, Excel, and Outlook.
- Ability to multitask and thrive in a fast-paced environment.
- Comfortable spending most of the day on the phone engaging with clients and families.
- Previous experience in senior care, healthcare, or social work is a plus.
- Experience with WellSky is a plus
